Choosing

Psychologist or psychiatrist:
who should you see first?

A psychiatrist is a medical doctor who can prescribe. A psychologist works through assessment and talking methods. Here is how to decide which one you need.

Counseling only

A psychiatrist is a medical doctor who can diagnose and prescribe medication. A psychologist works through assessment and talking methods and does not prescribe. For low mood, worry, anger or relationship difficulty, many people start with counseling; severe, worsening or safety-related symptoms need medical review as well.

The difference

What each professional does

01

Psychiatrist

A qualified medical doctor with specialist training in mental health. Can diagnose formally, prescribe and adjust medication, order investigations and admit a patient where necessary.

02

Psychologist

Trained in psychological assessment and talking methods. Works on patterns of thought, behaviour and relationships. Does not prescribe medication.

03

Counselor or therapist

Titles vary and are not uniformly regulated in Pakistan, so ask about qualifications and training rather than relying on the word used.

04

This practice

Counseling only, with psychologist Zahid Sohail. No prescribing, no medical certificates, no emergency cover.

Deciding

When to start with counseling

For relationship difficulty, anger, sustained stress, low confidence, grief or a decision you cannot settle, counseling is usually the more direct starting point. None of these needs a prescription, and all of them respond to being examined out loud with someone outside the situation.

Many people also start with counseling for mild to moderate low mood or anxiety, which is consistent with guidance such as NICE NG222 matching treatment intensity to severity.

When to see a doctor first

These are not borderline cases. In any of them, medical assessment comes before, or alongside, talking therapy.

  • Thoughts of ending your life, or of harming yourself or anyone else.
  • Symptoms that are severe, or worsening quickly.
  • Hearing or seeing things others do not, or losing touch with reality.
  • A sudden change in mood or behaviour after a head injury, illness or new medication.
  • Physical symptoms that have never been checked by a doctor.

Seeing both at once

Using both is common and often the recommended route: a psychiatrist managing medication, a psychologist working on the patterns. They are not competing approaches.

If you already take medication, counseling can run alongside it. Questions about starting, changing or stopping a prescription go to the doctor who issued it, not to a psychologist.

This practice will say plainly when a concern needs a psychiatrist, and will not offer an opinion on your prescription.

Questions, answered

Questions people ask.

Is it better to see a psychiatrist or a psychologist?

A psychiatrist is a medical doctor who can prescribe; a psychologist works through assessment and talking methods. For relationship difficulty, low mood, worry or anger, many people start with counseling; severe or worsening symptoms need medical review too.

Who should I see for anxiety, depression or stress?

Either professional is a reasonable starting point. Counseling suits people who want to understand and change a pattern. A psychiatrist is needed for diagnosis with medication, or where symptoms are severe, sudden or unsafe.

What is the difference between a psychologist and a psychiatrist?

A psychiatrist is a medical doctor who can diagnose and prescribe medication. A psychologist works through structured conversation and assessment and does not prescribe.

Which one do I need?

If you want to examine a pattern, a decision or a relationship, a psychologist is the usual starting point. If symptoms are severe, sudden or worsening, seek medical assessment first.

Can a psychologist give me a medical certificate?

No. A certificate, a formal diagnosis on paper or a prescription is a matter for a psychiatrist or a physician rather than this practice.

Can I see both?

Yes, and many people do. Medication and counseling address different parts of the same problem, and working with both at once is common rather than unusual.

Is Zahid Sohail a psychologist or a psychiatrist?

A psychologist. He holds an MSc and a PGD in Applied Psychology, works through conversation rather than prescription, and says plainly when a concern belongs with a doctor.

Who should I see first?

If you are unsure, the free 10-minute introductory call is a low-cost way to find out. Where a psychiatrist is the better fit, you will be told so rather than booked in.
