Published guidance and reviews report that talking therapy delivered by video or telephone can work comparably to in-person sessions for common concerns such as depression and anxiety, for many but not all people. It is less suitable for crisis situations, and no format guarantees a specific outcome.

The short answer

What does the evidence actually cover?

Research on remote talking therapy generally compares it to the same therapy delivered in person, most often for depression and anxiety, and most often by video or telephone. The broad finding across reviews is that remote delivery performs comparably to in-person delivery for many common concerns.

One of the more specific questions, whether the working relationship between client and therapist survives a screen, has been examined directly in a 2024 systematic review and meta-analysis in the Journal of Telemedicine and Telecare. That relationship is worth paying attention to, because it is what most people actually doubt.

What the evidence does not say

Being precise about this matters more than the headline.

  • It does not say online is better than in person.
  • It does not say every concern responds equally well.
  • It does not cover crisis care, and no study should be read as supporting remote management of immediate risk.
  • It says nothing about this practice specifically, or about your situation.

Fit

Where online sessions work well

01

Ongoing, non-urgent concerns

Low mood, worry, stress, anger and relationship patterns are the situations remote therapy is best studied in.

02

When privacy at home is manageable

A room with a door and headphones is usually enough. Where it is not, text may fit better than a call.

03

When travel is the obstacle

Distance, traffic, cost and being seen entering a clinic stop a great many people from starting at all.

04

When the language matters

Online access widens the choice of practitioner, which matters most when you want sessions in Urdu or Punjabi.

Where a different format is safer

Professional guidance on remote practice, including the APA’s telepsychology guidelines, expects a practitioner to judge whether remote delivery is appropriate for a given person and to say so when it is not.

  • Immediate risk to life. Emergency services or a hospital, now, not a scheduled session.
  • Severe or rapidly worsening symptoms needing medical assessment.
  • Situations where physical safety at home is the immediate issue.
  • Where no private space exists at all, and a call would be overheard by the person the session is about.

If any of these apply, this practice will say so on the free call rather than taking the booking.

What the research says

  • A 2018 systematic review comparing guided internet-based and face-to-face cognitive behavioural therapy found the two formats gave broadly equivalent results, while noting more research is still needed for some conditions. [1]
  • Talking therapies involve speaking with a trained professional about your thoughts, feelings and behaviour, and are used for a wide range of difficulties. [2]

Questions, answered

Questions people ask.

Is online counselling effective?

Reviews and public health guidance report that talking therapy delivered remotely can work comparably to in-person sessions for common concerns such as depression and anxiety, for many but not all people. Crisis situations need in-person or emergency care.

Can ChatGPT or an AI chatbot do therapy?

An AI chatbot can offer general information and is not a clinician. It cannot take clinical responsibility, cannot assess risk reliably and is not bound by professional confidentiality. Use it for information, not for care.

Is online counseling as effective as in person?

Published reviews generally find online delivery comparable to in-person for common concerns such as depression and anxiety. Outcomes vary by person and no practitioner can promise a result.

When is in-person counseling better?

When someone needs a physical assessment, when the situation is an emergency, or when a private connected space genuinely cannot be found. Those are practical limits rather than doubts about the method.

Does text counseling work as well as speaking?

It works differently. Writing suits reflection and people who cannot speak privately, but it removes tone and pace, and it is not suitable for urgent situations.

How soon would I notice a difference?

There is no reliable timeline, and anyone quoting one before hearing your situation is guessing. Sessions are booked one at a time so that you can judge for yourself.

What makes online counseling less effective?

Mostly practical things: an unreliable connection, a room where you can be overheard, or attending while doing something else. A private hour matters more than the format.

Can online counseling replace medication?

No. This practice does not prescribe, and counseling is not a substitute for medical treatment. If medication is worth considering, that is a psychiatrist or a physician.
